Application for Financial Assistance

THE R.V COLEMAN TRUST is a Charity providing relief in sickness for those residents of Dover and the immediate neighbourhood.

Are you or your relatives in need of respite care or assistance?

The R V Coleman Trust primarily provides financial assistance for convalescence and respite care, however other requests to assist those defined above will also be considered.

Whatever financial relief is provided under the scheme, however, must be related to the need of each case and be reasonable in the circumstances. Those who are sick or have special needs also includes those who are mentally and physically disadvantaged.

PLEASE NOTE:

1 THE TRUST IS UNABLE TO ASSIST THOSE LIVING OUTSIDE ITS SPECIFIED AREA OR THOSE IN RECEIPT OF STATE PENSION CREDIT OR GUARANTEED CREDIT.

2 Applicants/residents who are in receipt of PENSION CREDIT OR GUARANTEED CREDIT should refer to the local KCC Social Services department for support.

3 Individuals may receive financial assistance only once is a calendar year. In residential cases this relates to a total of two weeks’ care in the year.

4 All referrals for residential care should be made of the Trust’s referral form held at G.P surgeries. The completed form should be signed by the patient’s Medical Practitioner and either posted or emailed to the Trust’s Welfare Officer prior (if possible) to the proposed date of admission. The Home selected will then be contacted and advised that the Trust will pay a contribution towards its account less the patient’s contribution.

5 THE MAXIMUM AMOUNT THE TRUST WILL PAY IN RESPECT OF NURSING HOME CARE IS £1,000 p.w AND £1,250 p.w FOR RESIDENTIAL HOME CARE.

PATIENTS THEREFORE, WILL NEED TO PAY THE BALANCE (directed to the Home) WHICH WILL DEPEND UPON THE FEES CHARGED BY THE SELECTED NH/RH.

6 Repairs, adaptations or maintenance cannot be considered.
